Description

Dachely Hernandez is a family nurse practitioner passionate about helping patients take an active role in their health. She provides comprehensive family practice care for patients age 2 and up. Proud of her Cuban heritage, she is fluent in both English and Spanish, allowing her to connect with and serve a diverse patient population. Dachely earned her Doctor of Nursing Practice from Clemson University, where her doctoral project focused on developing a three-month CLABSI-centered educational program for medical-surgical nurses. During her doctoral studies, she served as a graduate teaching assistant and clinical instructor at Clemson University – a role that deepened her love for mentoring and education. She also holds a Bachelor of Science in Nursing from Florida State University. Her five-year nursing background has been primarily focused in medical-surgical areas. Her approach to care is patient-centered and evidence-based, rooted in the belief that empowering patients to take the driver's seat in their own health will lead to better outcomes. When she's not caring for patients, Dachely enjoys yoga, reading, baking and spending quality time with friends and family.
